glassFROGG is an online service that provides healthcare cost transparency and other healthcare related tools for the benefit of our users. We recognize our responsibility to protect the data that You entrust to us. We understand that providing information online involves a great deal of trust on your part. We take this trust very seriously, and make it a high priority to ensure the security and confidentiality of the personal information you provide to us when you visit our Site or use our services.
glassFROGG collects several types of Personal Information: personally identifying information that cannot be traced back to You; personally identifying information that can be traced back to You; and personally identifying information that can be traced back to You and that includes or is linked or associated with health-related information.
You can access and browse the Site at any time without providing Personal Information that can be traced back to You. To use some of its services and access certain areas of the Site, You must provide Personal Information that can be traced back to You, such as full name, street address and zip code, telephone number, e-mail, gender, date of birth, insurance coverage, social history and known health characteristics and medical needs. We collect this Personal Information that You voluntarily provide through the use of on-line data entry, survey, product purchase mobile applications and registration forms. glassFROGG gathers most of this Personal Information from You during an initial voluntary User registration process for the Site. glassFROGG also occasionally asks You for Personal Information at other times, including when You complete optional survey forms, fill out optional contact forms for additional information on particular products and services or use a particular service. Accordingly, You can determine to a large extent what Personal Information You wish to submit on a voluntary basis to glassFROGG. To participate in glassFROGG’s Healthcare Cost Transparency™ initiative (HCT™) simply follow the in-app or online prompts for submitting Your health care bills or statements. glassFROGG will use its best efforts to remove personally identifying information from submitted documents. Your documents will never be shown publicly.
When You visit the Site, glassFROGG may also automatically collect with Cookies, and similar technology, Personal Information about Your use of the Site. This Personal Information cannot be traced back to You. Cookies are small pieces of information that are stored by Your web browser software on Your computer's hard drive or temporarily in Your computer's memory.
glassFROGG also automatically collects the location of Your computer on the Internet, known as Your internet protocol address ("IP Address"), when You visit the Site. When you use an Application on a Device, we will collect and use information about you in generally similar ways and for similar purposes as when you use the Site. In addition, we may collect information about your location if you have instructed your Device to send such information to the Application via the privacy settings on that Device, or if you have uploaded photos tagged with location information. We may use the location information collected from your Device or photos to enhance your use of the Application by providing you with relevant content and contextual advertising. You can change the privacy settings of your Device at any time, in order to turn off the functionality to share location information with the Application and/or the functionality to tag your photos with location information. Please note that turning off location sharing may affect certain features of our Site. You must be 18 years of age or older to use the Site. If glassFROGG learns that it has received any Personal Information from anyone who is not at least age 18, We will delete that information from glassFROGG’s database.

Our Site also may use Web beacons (also known as clear gifs, pixel tags or Web bugs), which are tiny graphics with a unique identifier, similar in function to cookies, that are placed in the code of a Web page. Once the Internet industry has settled on standards related to this issue and glassFROGG recognizes such standards, we may reevaluate this approach.
With respect to obtaining certain products and/or services from this Site, glassFROGG may disclose to a third party Your Personal Information that We gather from You during an initial voluntary User registration process for the Site ("Registration Information"); as a result, You do not have to enter Your Registration Information more than once. glassFROGG may also disclose Your Registration Information to certain third parties in connection with various programs and tools created and administered by third parties that are included in the Site as an additional service to users. The use of Your Registration Information by these Third Party Programs is described below in the "What about information You enter into Third Party Programs included in the Site?" section.

glassFROGG discloses Personal Information to third parties or advertisers in aggregate form, that is, in a manner that cannot be traced back to You. For example, glassFROGG might tell an advertiser how many males between 20 and 30 years of age have visited the Site over a period of time.
glassFROGG may use and disclose Your Protected Health Information (i) for its internal management and administration; (ii) to carry out its legal responsibilities; and (iii) to perform certain data aggregation services for the Healthcare Provider and other Healthcare Providers; provided that, any disclosures for Our internal management and administration or to carry out Our legal responsibilities are either required by law or made after glassFROGG obtains reasonable assurances from the person to whom the Protected Health Information is disclosed that it will be held confidentially and used or further disclosed only as required by law or for the purpose for which it was disclosed to that person.
On this Site and within glassFROGG, We seek to use appropriate administrative, technical and physical security measures to reasonably safeguard Your Personal Information and Protected Health Information under Our control from unauthorized use, release or access. For example, glassFROGG grants access to any stored Personal Information and Protected Health Information only to authorized personnel. When You use the Premium Service or other glassFROGG services, You may receive various administrative e-mails automatically sent by glassFROGG. These administrative messages do not contain marketing-related information. You cannot opt-out from receiving any administrative e-mail sent by glassFROGG.
To comply with HIPAA, Your Healthcare Provider must provide You with rights in certain circumstances with respect to Your Protected Health Information. Very generally described, these rights are a right to restrict the uses and disclosures of, a right of access to, a right to amend and a right to receive an accounting of the disclosures of Your Protected Health Information. These limited rights will be described in detail in the Healthcare Provider's notice of privacy practices. If You wish to restrict the uses and disclosures of Your Protected Health Information, amend, or receive an accounting of the disclosures of Your Protected Health Information, then, You must do so through Your Healthcare Provider. Upon termination of Our Business Associate Agreement with a particular Healthcare Provider, glassFROGG generally must return or destroy all Protected Health Information received on behalf of or created for that particular Healthcare Provider and then maintained in any form by glassFROGG or a Subcontractor.

glassFROGG believes that it is important for You to easily differentiate between advertising and independent, editorial content on the Site; thus, glassFROGG will work to ensure that the advertising You see on the Site has no influence on the quality, accuracy or selection of editorial content that glassFROGG provides. glassFROGG clearly identifies all advertising on the Site. If you upgrade your account to the premium service, we will collect payment information from you, including your credit card number, cardholder name, expiration date, authentication code, and billing address. We will securely process the agreed to amount through our secure merchant portal
